The Commonwealth of Kentucky
Whereas, John Salyer has been appointed by the County Court of
Knott county, and qualified as Guardian to Allen Collins (illegible), a Minor:

NOW WE, John Salyer, as principal, and C. C.
Bates his sureties,
do hereby covenant to and with the Commonwealth of Kentucky, that the said
John Salyer will faithfully discharge the trust of Guardian to said Minor, in all
respects as required by law.
