State of Oklahoma County of Kiowa

On this 31 day of May, A. D. one thousand nine hundred and twelve, personally appeared before me, a Notary Public within and for the county and State aforesaid, WILLIAM G. HARRIS , who being duly sworn according to law, declares that he is 70 years of age, and a resident of Lone Wolf, Okla. County of Kiowa, State of Oklahoma; and that he is the identical person who was enrolled at Scottville, Kentucky under the same of WILLIAM G HARRIS, on the 23rd day of August, 1863, as a Private, in Company B 52nd Regiment of Kentucky Mounted Infantry Volunteers in the service of the United States, in the civil war and was honorably discharged at Bowling Green, Kentucky, on the 17th day of January, 1865.

That he is totally disabled by reason of gunshot wound in left leg received while in service and fistula contracted during the war.

That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 6 feet; Complexion, Fair; color of eyes, Blue ; color of hair, Dark; That his occupation was Farmer; that he was born March 9, 1842 at Simpson County, Kentucky.

That he several places of residence since leaving the service have been as follows: Scottsville, Ky, Halfway, Ky, Travis, Ky, until about 1907, Lone Wolf, Okla. until present date.

That he is a pensioner under certificate No. 6112185

That he makes this declaration for the purpose of being places on the pension roll of the United States under the provisions of the act of May 11, 1912.

That his post-office address is Lone Wolf, county of Kiowa, State of Oklahoma.
